Are you moving into or out of the area served by the Sammamish Plateau Water and Sewer District? The District needs certain information in order to stop or start service for our customers. Please click on the appropriate link below to open a form in Acrobat or Adobe Reader. When you are finished, click "Submit Form" and your completed form will be e-mailed to our Billing Department. Please note: We need at least one day's advance notice in order to process your request within a sufficient time frame. If you have already moved and have not notified us, we will make the changes on the date we receive notification.
